Preventive Maintenance Only

This plan covers one (1) preventive maintenance (PM) visit and which will include performance verification of the reader(s), pipettor and Turbidimeter. All labor, travel, expenses, and PM parts are covered. Coverage is Monday – Friday, 8:00 am – 5:00 pm. All workmanship (labor, travel, parts) is warrantied for 30 days upon completion of the PM. Telephone technical support and routine troubleshooting is included. EXCLUDES: On-demand service visits, non-PM parts, product enhancements and replacement or peripherals of any system components.

Full Service Plan

This plan includes all labor, travel, expenses and parts for an unlimited number of on-demand service visits. Includes one (1) preventive maintenance visits per year. Includes all replacement parts and modules as deemed necessary by our Technical Assistance Center and Field Engineers. Telephone technical support and routine troubleshooting is included. All product enhancements are included at no additional charge. Coverage is Monday – Friday, 8:00 am – 5:00 pm. The term of this contract is per calendar year, payable in advance.

Per-Call Plan

Under a per-call plan, a purchase order is required for each service visit. All parts, labor, travel and expenses are invoiced at the prevailing rates. When Biolog, Inc. provides replacement parts for equipment being serviced, list price is charged. Partial credit is available for returned parts which are repairable. There is a minimum charge of 2 hours labor and 2 hours travel, plus actual expenses (e.g. airfare, hotel, rental car) at the prevailing labor and travel rates. Biolog, Inc. business hours are Monday - Friday from 8:30 am – 5:00 pm PST.

On-Site Validation Package of the Biolog
OmniLog® ID System

Service Description for U.S. and Canada only

This on-site validation covers the Biolog OmniLog® ID system performance specifications at one site. A Biolog representative will perform all the activities described in Biolog’s Validation Protocol Manual. The hours during which these activities will be completed are Monday – Friday, 8:00 am – 5:00 pm. This package includes all labor and travel expenses of the Biolog representative. This service includes validation of the gram-negative and gram-positive databases. The customer must allow the Biolog representative access to all materials for 4 days to complete the validation. * The customer is responsible for any changes, additions or modifications to Biolog’s Validation Protocol Manual.

On-Site Validation Package of the Biolog
OmniLog® Plus System

Service Description for U.S. and Canada only

This on-site validation covers the Biolog MicroStation and OmniLog® ID system performance specifications at one site. A Biolog representative will perform all the activities described in Biolog’s Validation Protocol Manuals. The hours during which these activities will be completed are Monday – Friday, 8:00 am – 5:00 pm for two consecutive weeks. This package includes all labor and travel expenses of the Biolog representative. This service includes validation of the gram-negative, gram-positive and yeast databases. The customer must allow the Biolog representative access to all materials for 9 days to complete the validation. * The customer is responsible for any changes, additions or modifications to Biolog’s Validation Protocol Manuals.
Note: If the customer is validating the Fungal database (FF) they must purchase Biolog FF QC Strain Set. Biolog will initiate the setup and reading of the organisms.

On-Site Validation Package of the Biolog MicroStation™

Service Description for U.S. and Canada only

This on-site validation covers the Biolog MicroStation system performance specifications at one site. A Biolog representative will perform all the activities described in Biolog’s Validation Protocol Manual. The hours during which these activities will be completed are Monday – Friday, 8:00 am – 5:00 pm. This package includes all labor and travel expenses of the Biolog representative. This service includes validation of the gram-negative, gram-positive and yeast databases. The customer must allow the Biolog representative access to all materials for 4 days to complete the validation. * The customer is responsible for any changes, additions or modifications to Biolog’s Validation Protocol Manual. *Note: If the customer is validating the Fungal database (FF), they must purchase the Biolog FF Quality Control Strain Set, catalog number 8046. Biolog will initiate the setup and reading of the organisms, but the customer will be responsible for completion of the subsequent readings.
